| Sculptra has been in use for over 5 years in 33 countries under
the trade name of New-Fill. New-Fill is used in these countries
in general aesthetic practice to fill deeper wrinkles and skin defects.
New-Fill has been in use for 4 years also to specifically treat
HIV lipoatrophy of the face, the sunken cheek syndrome
which often stigmatizes persons with HIV disease.
Dr. Engelhard trained in Paris with Dr. Elisabeth LaGlenne, the
founder of New-Fill. Dr. Engelhard was the first U.S. doctor authorized
by the FDA to import and administer New-Fill, and Dr. Engelhard
began studies in 2001, with remarkable results in patients with
HIV Lipoatrophy (facial fat loss). These studies were critical
to the FDA application for this product. New-Fill is now marketed
under the trade name Sculptra, and is available in the U.S. as
of August 2004.
In the treatment of facial lipoatrophy, a series of 3 to 6 treatments
at monthly intervals are needed to see the best results. Sculptra
is composed of tiny crystals of polylactic acid (PLLA), and when
these crystals are injected into the deep layers of the skin,
collagen forms in response to the PLLA. This response is slow
and gradual, and the best effects of Sculptra treatment are usually
seen a few months after the last treatment session. The number
of treatments required depends on the severity of the existing
fat loss and the desired cosmetic effect. Touch up usually requires
only a single treatment every year or two. The PLLA crystals and
the collagen formed slowly reabsorb over the course of a few years.
